IGNITE - St. Paul's Youth Group - Parent Consent Form (2025/2026)

IGNITE is a high school youth group at St. Paul the Apostle Church.   It is a Christ-centred youth group that welcomes teens from grade 9 to grade 12.  We generally meet on the 1st and 3rd Fridays of the month (with some exceptions), and from 7-9pm.

Your teenager has expressed a desire to join the IGNITE youth group.  Please take a few minutes to register your teen by completing this form.

We look forward to journeying with your teenager on his/her journey with Christ!

Note : the drop-off and pick-up is from Room E, downstairs (access from the outside, below-ground patio/courtyard door, facing Taylor Kidd Blvd).

I hereby GRANT permission for my child to be photographed and/or videotaped during youth ministry related activities and events. I understand that my child may decline to be photographed and/or videotaped at any time.
I further GRANT permission for the resulting photographs and/or video footage to be edited, if necessary, and then published (on Parish-related Social Media and/or Posters) for the purposes of promoting our youth programs here at St. Paul's Parish.

I consent to my child (listed on this form) participating in St. Paul the Apostle's High School Youth Group (IGNITE).

I/We understand that reasonable precaution will be taken to safeguard the health and safety of the participant and that the designated emergency contact person will be notified as soon as possible in case of emergency. In the event of any sickness or accident, person(s) will not hold St. Paul the Apostle Parish, the Archdiocese of Kingston, any volunteer, chaperone, or driver responsible.
I/We authorize and consent that emergency treatment be rendered under the general or specific supervision and on the advice of any physician, dentist, or surgeon; licensed to practice in the Province of Ontario or any other Province. The undersigned understand(s) and agrees that any medical, dental, or hospital expense incurred shall be at their own risk. The undersigned understand(s) every effort will be made to notify the emergency contact in the event that treatment is necessary.

St. Paul's HS Youth Group's Communication Policy with Minors (informational only)

A priority of our mission within the High School Youth Ministry is to help lead young people into a relationship with God, and to create a community of friends and believers among each other. Since so much of the teens’ communication happens through technology, we have found it helpful in our ministry to be able to reach out to teens in different ways.

To increase our ministry outreach during this academic year, an adult core team member may reach out to your student(s) through a variety of technology platforms which are listed below.

Our policy is to have two or more core team members included in any group or individual communication with students at all times. All communication by our core team members will abide by Archdiocesan policies with behaviour between adults and minors (Safe Environment Policy and Guidelines for Parish Volunteers). This is to protect both our students and our core team members.

If a student should reach out one-to-one (1:1) to an adult core team member through electronic means, that core member will communicate back something along the lines of:
“I have received your message but am unable to reply as you have not copied an additional core team member and/or parent on this message. Please resend to me with at least one of these options so I can reply to you. If this is an emergency and you need to communicate with me immediately, reply and include that in your message. Thank you!”
“I have received your message but am unable to reply individually as you have not copied an additional core member and/or parent on this message. I have included {Core Team Member’s Name} with this reply.”

Digital means used to communicate with your teen may be any of the following:
- email
- Social Media (through St. Paul's Youth Ministry social media presence - Instagram, Facebook, etc).
- text message
